The List for Admission
1. Application Form for Admission Chongqing City Management College (please typewrite, don't fill in it by pen, otherwise we can't understand you very clearly); please check the attached file.
2. Passport photocopy;
3. Working or Studying Certificate;
4. Highest Diploma and transcript with notarized;
5. Study or research plan in Chinese or in English
6. Certification of no criminal conviction;
7. Foreigner's Physical Examination Form (refer to annex) and blood test report. Original forms must be presented when you come to register. The medical examinations must cover all the items listed in the Foreigner's Physical Examination Form. Incomplete records or those without the signature of the attending physician, official stamp of the hospital or a seal photograph of the applicants are invalid..

2、护照个人信息页复印件；
3、高中（或以上）毕业证复印件；
4、高中学习成绩单复印件；
5、英文或汉语自我介绍一份（500字左右）；
6、体检表复印件（在中国大使馆指定体检机构进行体检）；
7、无犯罪记录原件一份；
8、HSK3证书（语言生免提供）；
9、近期电子护照照片。
